What Is TMJ — and Why Is It So Hard to Treat?

 

Temporomandibular joint disorder (TMJ or TMD) refers to dysfunction in the joint that connects your jaw to your skull. The symptoms can range from jaw pain, clicking, and popping to chronic headaches, facial tension, ear pain, and even dizziness. Many people with TMJ also report difficulty sleeping, anxiety, and brain fog — symptoms that seem unrelated on the surface but are deeply connected.

 

The challenge with TMJ is that most treatments focus only on the jaw itself. Mouthguards protect your teeth from grinding but don’t address why you’re grinding in the first place. Dental appliances reposition the jaw, but if the underlying structural or neurological trigger isn’t corrected, symptoms tend to return.

 

The Upper Cervical Connection Most Dentists Don’t Mention

 

The upper cervical spine — specifically the atlas (C1) and axis (C2) vertebrae at the very top of your neck — sits directly beneath your skull and adjacent to the temporomandibular joint. A misalignment in this region can have a cascade of effects on the jaw:

 

First, the atlas supports the skull. When it shifts even a fraction of a millimeter, the skull can tilt, which alters how the lower jaw sits and functions. This creates uneven stress on the TMJ that no amount of dental work will fully correct.

 

Second, the trigeminal nerve — which controls sensation and movement in the jaw — passes through the upper cervical region. Misalignment can irritate this nerve, contributing to facial pain, involuntary clenching, and bruxism (teeth grinding).

 

Third, and perhaps most importantly, the brainstem is housed right at the craniocervical junction. Structural stress here can dysregulate the autonomic nervous system, increasing muscle tension throughout the head, neck, and jaw — the exact tension pattern that drives so many TMJ flare-ups.

When we take a detailed imaging series and analyze the alignment of the atlas and axis, we frequently find significant misalignment that has gone undetected.
